We provide a comprehensive range of medical and psychiatric services, including talk therapy, medication management, anxiety and depression care, ADHD evaluations, and integrative mental health support. Coming soon, we will also offer weight management/loss programs, hormone replacement therapy, and sexual health services.


Controlled medications may be prescribed when clinically appropriate, as part of a safe and effective treatment plan. Our providers carefully evaluate each patient’s needs and follow strict guidelines when considering controlled substances.

  • Prescribing Policy:
    Controlled substances are only prescribed when deemed medically necessary. Patients receiving these medications are required to follow up every 30 days and adhere to all practice policies.
  • Safety Measures:
    For patients who require more than one controlled medication, we believe it is safest for your care to be managed in person. In such cases, we will refer you to an in-person provider for ongoing treatment.

Our services are currently available to adults aged 18 and older.


We currently serve patients in Westchester County, all counties in Long Island, and all five boroughs of New York. In the near future, we will be expanding to New Jersey, Connecticut, and Massachusetts, with plans to offer services in all 50 states.


No. Due to licensing regulations, you must be physically located in the state where your psychiatric nurse practitioner is licensed at the time of your session. For example, if your care was initiated in New York, you must be in New York during your appointments. If you temporarily reside in another state, such as a student spending the summer elsewhere, we are only able to continue your care if we have a licensed provider in that state and can arrange a temporary reassignment. Otherwise, we are unable to provide treatment outside your provider’s licensed state.


At Top Tier Health and Wellness Group, your safety and well-being are our top priorities. Some conditions require specialized care and additional resources beyond the scope of our practice. For this reason, we do not provide treatment for:

  • Individuals under the age of 18
  • Active suicidal thoughts or self-harm
  • Recent psychiatric hospitalization
  • Schizophrenia, schizoaffective disorder, or other psychotic disorders
  • Dissociative Identity Disorder
  • Personality disorders (including borderline personality disorder)
  • Eating disorders (anorexia, bulimia)
  • Substance use disorders (active)
  • Court-ordered treatment
  • Autism spectrum disorders
  • Non-verbal learning disorders
  • Mixed receptive-expressive language disorders
  • Traumatic brain injury (TBI)
  • Epilepsy or other seizure disorders
  • Cardiac arrhythmia

We provide 100% convenient and confidential virtual tele-health for convenience and accessibility to access care from the comfort and privacy of your home.


We accept a variety of insurance plans. Please contact our office or visit our website for the most up-to-date information regarding accepted insurance carriers. Medicaid and Medicare are not accepted.


Our psychiatric nurse practitioners primarily focus on medication management and incorporate therapy-informed interventions into your treatment. For those needing ongoing therapy, we collaborate with your existing therapist or can provide referral options to ensure comprehensive care.

Anxiety disorders can manifest in various ways. Common signs include persistent worrying, restlessness, trouble concentrating, and physical symptoms like a racing heart or sweating. If these symptoms interfere with daily life, it might be time to seek support.


While sadness is a natural part of life, depression is more intense and persistent. Symptoms include feeling hopeless, a lack of energy, changes in sleep or appetite, and loss of interest in activities once enjoyed. Research has shown, If these feelings last more than two weeks, consider reaching out to Top Tier Health and Wellness Group for help.


Many STDs might not show symptoms immediately, but when they do, they can include unusual discharge, burning during urination, sores or bumps around genitals, or itching. Regular testing and open communication with partners are key to maintaining sexual health.
